School Packet 2010-11 - MS/US

This is a list of forms needed for the 2010-2011 School Year.
Please refer to the Documents & Forms page throughout the year for all forms.

Middle & Upper Schools Forms:

First - Download the MS/US Checklist.  This will assist you in ensuring you have everything you need to enroll in grades 6-12.

Immediate dates you need to know:

• Saturday, August 7, 9AM-12PM, School Physcials for grades 6-12, $10, ASA Gym
• Monday, August 9, 11AM-1PM, Book Distribution - Grades 7-9
• Tuesday, August 10, 11AM-1PM, Book Distribution - Grades 10-12 
• Tuesday,   August 10, 6:30-8:00PM - Grade 6 Book Distribution, Orientation & Dessert Party
• Wednesday, August 11, Grade 6 starts school, 1/2 day - 8AM-12PM
• Thursday, August 12, Grades 7-12 start school

Forms - DUE July 15, 2010, to Mrs. Heather Brown

• Student Emergency Card & Proof of Insurance Form (grades 6-12) 
• Authorizaton for Medication (for Rx medicine, if necessary)
• Medical Treatment Authorization (grades 6-12) (used for field trips, etc.)
• Technology Acceptable Use Policy
• Photography Release Form  - (new students only)
• Consent and Release from Liability (FHSAA) (EL03)*
• Physical Evaluation Form (FHSAA) (EL02)*
• Bus Transportation Sign-up Form - additional Lakeland pickup added on 8/13/10
• Very Important Grandparents Form
• Florida Certification of Immunization (Blue form, if entering 7th grade only)

Book Distribution - Hollis Media Center

• Grades 7-9 - Monday, August 9, 11AM - 1PM
• Grades 10-12 - Tuesday, August 10, 11AM - 1PM
• Grade 6   - Tuesday, August 10, 6:30-8:00PM - includes Orientation & Dessert Party

Dress Code Information

• Middle School (grades 6-8)
• Upper School (grades 9-12)
• P.E. Uniforms (grades 6-8)- must be purchased from SEO sports. See the order forms below.

Locker Information

•  All Middle and Upper School students are required to supply their own locks for lockers.
• Students will be assigned a locker during book distribution in August.

Lunch Information

• There will be no August lunches.  Students must bring lunch from home.
• September lunch form is due with payment in Mack Hall by 4PM, August 18.
• Lunch forms will be published monthly on the first day of the month beginning in September for the October Lunches.

    Three ways to get the lunch form:

1. Students may pick up a copy in the Mack Office from Mrs. Heather Brown.
2. Forms may be downloaded from the event published the first day of the month on the ASA website calendar.  The event will be titled,"MS/US Lunch Form Published" with a hyperlink to the form.  This will begin September 1.
3. Go to the Documents & Forms page on the website, and find the form under "Lunch Forms."
