School Packet 2010-11 - MS/US

This is a list of forms needed for the 2010-2011 School Year.
Please refer to the Documents & Forms page throughout the year for all forms.

Middle & Upper Schools Forms:

First - Download the MS/US Checklist.  This will assist you in ensuring you have everything you need to enroll in grades 6-12.
Immediate dates you need to know:
  • Saturday, August 7, 9AM-12PM, School Physcials for grades 6-12, $10, ASA Gym
  • Monday, August 9, 11AM-1PM, Book Distribution - Grades 7-9
  • Tuesday, August 10, 11AM-1PM, Book Distribution - Grades 10-12 
  • Tuesday,   August 10, 6:30-8:00PM - Grade 6 Book Distribution, Orientation & Dessert Party
  • Wednesday, August 11, Grade 6 starts school, 1/2 day - 8AM-12PM
  • Thursday, August 12, Grades 7-12 start school

Reminder:  School physicals for grades 6-12 are offered
Saturday, August 7, 9 AM-12 PM at ASA; $10/student. 
The FHSAA forms may be completed at this time*.

Book Distribution - Hollis Media Center

  • Grades 7-9 - Monday, August 9, 11AM - 1PM
  • Grades 10-12 - Tuesday, August 10, 11AM - 1PM
  • Grade 6   - Tuesday, August 10, 6:30-8:00PM - includes Orientation & Dessert Party

Dress Code Information

Locker Information

  •  All Middle and Upper School students are required to supply their own locks for lockers.
  • Students will be assigned a locker during book distribution in August.

Lunch Information

  • There will be no August lunches.  Students must bring lunch from home.
  • September lunch form is due with payment in Mack Hall by 4PM, August 18.
  • Lunch forms will be published monthly on the first day of the month beginning in September for the October Lunches.

    Three ways to get the lunch form:

  1. Students may pick up a copy in the Mack Office from Mrs. Heather Brown.
  2. Forms may be downloaded from the event published the first day of the month on the ASA website calendar.  The event will be titled,"MS/US Lunch Form Published" with a hyperlink to the form.  This will begin September 1.
  3. Go to the Documents & Forms page on the website, and find the form under "Lunch Forms."

   Just a reminder that students need to complete their summer reading.
